Translating the Canadian Occupational Performance Measure to Danish, addressing face and content validity

The Canadian Occupational Performance Measure (COPM) is a recognized assessment tool within Danish occupational therapy. This newsletter presents a rigorous translation and a cross-cultural validation applied to the 5th edition COPM.

The study (Larsen, Morville, and Hansen, 2017) consisted of an iterative, multistage translation with forward translation involving professional and bilingual occupational therapists (OTs), pilot testing and cognitive debriefing interviews with 15 OTs and 37 clients which were analyzed quantitatively and qualitatively, and finalization with specific comments, back translation and approval.

Content validity of the Danish version of the COPM regarding the appropriateness, acceptability, clarity and comprehensiveness was found in 78-100% of the cases. The COPM ensures a client-centred focus in the assessment process when the therapist focuses on occupation rather than specific activities. However, issues concerning the administration and content were identified. First, OTs had a tendency to implement the occupational therapy intervention processes during the interview, which is not the intent of the COPM assessment and second, clients who had not performed the occupation in their current condition were difficult to score. As a result, two recommendations regarding the administration of the COPM were added to the Danish version. The conclusion of the study is that semantic, conceptual, operational and aspects of item equivalence of the COPM translation into Danish were achieved and supported by face and content validity.
